In short: to fill in the EX11 form (version under RD 1155/2024) write in block capitals with a black pen or on a typewriter, and file original plus copy. It covers «larga duración» and «larga duración-UE» residence. It is free, and there is no official English version at inclusion.gob.es (checked 2026-10-05).
In person or by representative? Mixed: file at the Oficina de Extranjería or through Mercurio, also via a representative (section 2); the TIE card, in person.
1) DATOS DE LA PERSONA EXTRANJERA
- «PASAPORTE», «N.I.E.», «1er Apellido», «2º Apellido», «Nombre», «Nacionalidad», «Nombre del padre», «Nombre de la madre»: passport, NIE, surnames, name, nationality, parents' names.
- «Sexo»: H man, M woman, X undefined (only if your country of origin recognises it). «Estado civil»: S single, C married, V widowed, D divorced, Sp separated.
- «Fecha de nacimiento»: dd/mm/yyyy; «Lugar», «País»: place, country.
- «Domicilio en España» («Nº», «Piso», «Localidad», «C.P.», «Provincia»), «Teléfono móvil», «E-mail»: address (number, floor, town, postcode, province), mobile, email.
- «Representante legal (menor/tutelado…), en su caso»: legal representative, with «DNI/NIE/PAS» and «Título».
- «Hijas/os a cargo en edad de escolarización en España»: SÍ or NO (children in your care of school age).
2) DATOS DEL REPRESENTANTE A EFECTOS DE PRESENTACIÓN DE LA SOLICITUD: only if someone else files while you are in Spain («Nombre/Razón Social»: name or company; «DNI/NIE/PAS» and contact details). Representation must be proven with a notarial power of attorney or apud acta (Sheet 60; art. 197.4 RD 1155/2024).
3) DOMICILIO A EFECTOS DE NOTIFICACIONES: the same fields, for notices; if left blank, notices go to the applicant. The «CONSIENTO…Dehú» box (electronic notices) requires a digital certificate or Cl@ve.
4) TIPO DE AUTORIZACIÓN SOLICITADA: tick the type of authorisation that applies.
- «RESIDENCIA LARGA DURACIÓN»: general case, 5 years of continuous residence; contributory retirement pension, or absolute permanent incapacity or gran invalidez; born in Spain, over 18, 3 years of prior residence; Spaniard by origin who lost nationality; ward of a public entity; stateless person or refugee (also subsidiary protection: Sheet 49, art. 183.3.f); renewal for a reunified relative; EU long-term holder in another member state; recovery.
- «RESIDENCIA LARGA DURACIÓN-UE»: general case, 5 years, with income and health insurance; earlier study, exchange or non-work internship stay, counted at 50%; EU Blue Card; holder in another member state who gives it up; recovery.
Mistakes to avoid: «CONSIENTO» without a certificate or Cl@ve; the EU option without proof of income and insurance.

Steps:
- Download the official PDF or the editable version of the EX-11.
- Gather the documents listed in Sheet 49 (long-term) or Sheet 50 (long-term EU). Both ask for a complete copy of a valid passport (or travel document or registration certificate), proof of fee payment and, where applicable, a criminal record certificate. Sheet 49 adds, for applications based on earlier periods of residence, the school enrolment report for children in your care of compulsory school age, and the EU one adds proof of income and health insurance. The full list is in each sheet; foreign documents need a sworn translation and legalisation or apostille, unless exempt by treaty.
- Fill in sections 1 to 3 and tick the option in section 4 (general case: art. 183.1 for long-term and art. 176 for EU; the form labels it «art. 176.1.a»).
- Write place and date, sign in «FIRMA DEL SOLICITANTE (o representante legal, en su caso)» and complete «DIRIGIDA A», «Código DIR3» and «PROVINCIA». The form does not explain where the DIR3 code comes from: ask at the office.
- File original and copy at the Oficina de Extranjería of your province (appointment) or through Mercurio; from outside Spain, in person at the Spanish consulate of your area.
- Pay the fee: form 790, code 052, heading 2.6. According to the sheets, it falls due when the application is admitted for processing and is paid within 10 working days, but they also list the payment proof among the documents: confirm at your office when to hand it in.
- Wait for the decision. According to Sheets 49 and 50: 3 months and, with no notification, it may be deemed approved by silence. Exception: if you already hold EU long-term residence in another member state and apply for long-term residence in Spain (art. 179), the term is 2 months and silence means refusal (art. 179.4 RD 1155/2024). Once granted, request the TIE in person within 1 month.
