Important: these forms exist only in Spanish. Spain's Ministry of Inclusion does not publish an official English (or any other language) version of the PDF itself. This page explains, in English, what each form is and what it's for — but you (or a professional/translator helping you) will need to fill it in in Spanish.

Warning — form numbers that no longer exist: since the RD 1155/2024 reform took effect (20 May 2025), the codes EX-05, EX-08, EX-12, EX-14, EX-27 and EX-30 were removed or merged into other forms (e.g. the old EX-12 work-authorisation form is now folded into current EX-03). If you find a PDF online using one of these old codes, it's likely outdated — always download from inclusion.gob.es.
In person or by representative? The form itself doesn't require you to appear in person — you (or a representative, such as a gestor or lawyer) can fill it in. What may require your presence is the procedure the form is submitted for (e.g. fingerprinting for the TIE) — check that specific procedure's page or the Mercurio (online filing) guide.
Watch out for third-party websites: these forms are 100% free at inclusion.gob.es. Be wary of sites that charge a fee to "fill in" or "deliver" the same official PDF.
