In short: the health insurance for Spain visa applications is not the same for every visa. The rules ask for health insurance in non-lucrative residence, student stay, international teleworking and family reunification, but describe what the policy must look like only for students and teleworkers. The expressions «no copays» and «no waiting periods» do not appear in the rules: they come from consular practice.
In person or by representative? The rules do not regulate how to buy the policy: that depends on the insurer. The visa is filed as described in each visa card (links below), always at the consulate that covers you.
What the rules say, by visa type (RD 1155/2024 and the 2023 joint Instruction):
- Non-lucrative residence (art. 61.2.b): «health insurance» is a requirement, with no further conditions in the text. At renewal you must have kept it and continue with it (art. 64).
- Student stay (art. 35.i): insurance taken out with an insurer authorised to operate in Spain, with benefits similar to the basic common portfolio of the National Health System and valid for the whole planned stay. If you work and are registered with Social Security, it is treated as met (art. 57.5). If relatives join you on a higher-education stay, they also need health insurance (art. 56.3).
- International teleworking (Instruction, point five): public or private insurance with an insurer authorised in Spain, active for the whole authorisation. Travel insurance is not valid. If no international coordination rule covers you and you therefore have to pay into Spanish Social Security, a commitment to comply with Social Security obligations is enough (instruction five.2 and six). If an international agreement covers you, you prove it with a certificate from the competent institution. If the agreement does not provide health cover, you need insurance with cover comparable to the national health system.
- Family reunification (art. 67.3): the sponsor, who lives in Spain, must have health insurance for themselves and for the relatives listed in art. 66. The article says nothing more about the policy.
What the rules do not say. «No copays» (you pay no share of each medical act) and «no waiting periods» (no months of waiting before cover starts) are not in the Royal Decree or in the Instruction. «Authorised insurer» appears for students and teleworkers, not in the text on non-lucrative residence or family reunification. Cover similar to the public system appears for students and, for teleworking, only if the international agreement provides no cover.
How consulates apply it in practice (not a rule). Several consulates ask for a policy from an insurer licensed in Spain, with full cover, no copays, no waiting periods and valid for the whole period, plus an insurer's certificate. We could not confirm this on an official page and it may vary between consulates. Find your consulate's page in the Exteriores consular services search and confirm in writing which policy they accept before paying for it.
Versus Schengen travel insurance: the short-stay one covers a trip; for a residence visa the rules ask for health insurance. It must cover the whole stay for students and the whole authorisation for teleworkers; for non-lucrative residence the text only says «health insurance» (at renewal they check that you kept it) and for family reunification it does not specify the period. For teleworking, the rules also say travel insurance is not valid.
If you already have the right to public healthcare, see the health card. For the visa, Social Security registration counts for students, and the Social Security or agreement route counts for teleworkers. For non-lucrative residence and family reunification the rules do not say whether public insurance is enough: ask the consulate.
Steps:
- Identify your visa type and read its card (links above).
- Find your consulate's page and see which policy it accepts.
- Ask the insurer for a certificate that shows what your case requires: cover in Spain, authorised insurer, validity and, if the consulate asks, no copays and no waiting periods.
- Keep the policy active: it is checked again at renewal (for students and teleworkers it must cover the whole stay or the whole authorisation).
