In short: the EX-01 (also written EX01) is the official form for non-lucrative residence (living in Spain without working). No official English version exists, so the fields are translated here. Current version «03», under LO 4/2000 and RD 1155/2024. Free at inclusion.gob.es.
In person or by representative? Mixed. You can fill it in yourself; section 3 is for whoever files it for you. The initial application goes to the Spanish consulate (usually in person). The renewal goes to the oficina de extranjería in Spain (art. 64.1 RD 1155/2024), in person or online; the Supreme Court annulled the online-only rule in 2026 (art. 197.2), though note 6 on the form still cites it.
1) DATOS DE LA PERSONA EXTRANJERA SOLICITANTE
- PASAPORTE, N.I.E., 1er Apellido, 2º Apellido, Nombre: passport and foreigner ID numbers, surname 1, surname 2, first name.
- Sexo: H male, M female, X undefined (only if your country of origin recognises it).
- Fecha de nacimiento, Lugar, País: date, place and country of birth.
- Nacionalidad, Estado civil, Nombre del padre, Nombre de la madre: nationality; S single, C married, V widowed, D divorced, Sp separated; father's and mother's names.
- Domicilio en España, Nº, Piso, Localidad, C.P., Provincia, Teléfono móvil, E-mail: address, number, floor, town, postcode, province, phone, email.
- Representante legal, en su caso, DNI/NIE/PAS, Título: legal representative if any: ID, title (parent of minor, guardian).
- Hijas/os a cargo en edad de escolarización en España: school-age dependent children in Spain, SÍ yes, NO no.
2) DATOS DEL FAMILIAR TITULAR DE LOS RECURSOS ECONÓMICOS (EN SU CASO): only if a family member holds the funds. Its fields: PASAPORTE, N.I.E., apellidos, Nombre, Sexo, Estado civil, Fecha de nacimiento, País, parents' names and Parentesco (relationship). No Lugar, Nacionalidad, address or contact.
3) DATOS DEL REPRESENTANTE A EFECTOS DE PRESENTACIÓN DE LA SOLICITUD: only if not the applicant. Nombre/Razón Social (name or company), ID, address, contact, legal representative, Título.
4) DOMICILIO A EFECTOS DE NOTIFICACIONES: who receives notifications (if empty, the applicant). The CONSIENTO box moves them to the Dehú (electronic mailbox) and needs an electronic certificate or Cl@ve.
5) TIPO DE AUTORIZACIÓN SOLICITADA: RESIDENCIA INICIAL (initial) or RENOVACIÓN (renewal) and, below, Titular de los recursos económicos or Familiar del titular de los recursos económicos (holder or family member). Then place and date, FIRMA DEL SOLICITANTE and, below, DIRIGIDA A (addressed to), Código DIR3, PROVINCIA.
Mistakes to avoid: X under Sexo without recognition in your country of origin; CONSIENTO without a certificate or Cl@ve; section 3 filled in when you file alone.

Steps:
- Download the official PDF or its editable version at inclusion.gob.es. It is free and selling it is prohibited.
- Fill in section 1; section 2 only if the holder of the funds is a family member and section 3 only if someone else files. Write in capital letters with a black pen and block lettering, or type it, and dates as dd/mm/aaaa.
- If unsure what to enter in N.I.E. (no number yet), Domicilio en España (living abroad) or DIRIGIDA A and Código DIR3, ask at the consulate (initial application) or the oficina de extranjería (renewal): the form does not say.
- Enter the address for notifications and tick CONSIENTO only if you have an electronic certificate or Cl@ve; the certificate must be linked to the DNI/NIE shown in that section (note 6). Fill in Teléfono móvil and E-mail too: the form says they let you receive an informational alert about pending notifications.
- Tick the type of authorisation, add place and date, and sign.
- Prepare the original and a copy and hand in the form with the other documents. The initial application goes to the consular office (see non-lucrative residence). For the renewal, note 6 on the form says «only by electronic means», but the Supreme Court annulled that requirement (art. 197.2, ruling of 8 July 2026; see ruling on art. 197.2): it is filed at the oficina de extranjería, in person or online (Mercurio); details in renewal of authorisations.
