In short: the EX00 form (EX-00, version "03", LO 4/2000 and RD 1155/2024) is the application for a long-term stay (estancia de larga duración) for study, training or volunteering. No official English version found (per our EX forms guide), so field names stay in Spanish, with their meaning.
In person or by representative? Per RD 1155/2024: from abroad, the visa is requested at the consulate, in person or through a representative, and includes the authorisation request (art. 36.1). The consulate may summon you in person or for an interview; if you do not attend within the set time (15 days at most), the application is treated as withdrawn (art. 27.3). From Spain, in person, through a representative or electronically, at the immigration office of the province where the activity starts (art. 54.3). This route is for people who already hold a residence authorisation or, in cases such as higher studies or specialised health training, who are lawfully in Spain and of legal age (art. 54.1). Extension: electronic only (art. 55.4).
- 1) Datos de la persona extranjera: Pasaporte, N.I.E., 1er Apellido, 2º Apellido, Nombre (surnames, first name). Fecha de nacimiento, Lugar, País (date, place, country). Nacionalidad. Nombre del padre, Nombre de la madre (parents). Domicilio en España (address). Sexo: H male, M female, X undefined (only if you have it recognised in your country of origin). Estado civil: S single, C married, V widowed, D divorced, Sp separated. Representante legal, en su caso (if any): Título (basis of representation).
- 2) Institución/centro de estudios, formación o voluntariado: Denominación, NIF, Código, Dirección. Registro o reconocimiento oficial: RUCT (universities, centres, degrees), RCD (non-university schools), OTRO, Adscripción a Universidad or Otro reconocimiento oficial (DT única); say which in the last three.
- 3) Programa de estudios o formación: Denominación, Código, Fecha de inicio, Fecha de finalización. Modalidad: Presencial or Híbrida/Semipresencial (blended: university rules, or at least 50 % in person in other studies).
- 4) Familiar estudiante al que acompaña: Parentesco (relationship). 5) Empleador/a (employer): only for work for an employer. 6) Representante a efectos de la presentación: only if another person files.
- 7) Domicilio a efectos de notificaciones: the CONSIENTO box accepts electronic notices (Dehú; needs an e-certificate or Cl@ve); compulsory for legal entities and some groups (art. 14 Ley 39/2015).
- 8) Tipo de autorización de estancia solicitada: the INICIAL box, then one of these. Higher studies (52.1.a; Situación regular en España or Solicitada por institución), post-compulsory secondary (52.1.b), secondary exchange (52.1.c), volunteering (52.1.d). Also: training activities (52.1.e), specialised health training (58), mobility (59.2, 59.3), Andorra agreement, family (56, 56.7). Apart: PRÓRROGA (extension, 55.4); AUTORIZACIÓN PARA TRABAJAR (57.1): Cuenta ajena (employed) or Cuenta propia (self-employed).
Watch out: for a PhD apply for the research authorisation (DGGM Criterion 2/2026), not this stay.

Steps:

- Fill in the sections that apply to your case (1 to 7), in capital letters, with a black pen and block print (or typed), dates as dd/mm/aaaa. Sections 4, 5 and 6 are filled in only in the cases the form itself describes.
- In section 8 tick the option that fits your procedure (requirements, documents and fees: its information sheet). Several options carry the printed condition «Titular de autorización de residencia en España» or «Situación regular»: read it before ticking.
- Write place and date, sign (you or your legal representative) and complete «Dirigida a» with the body, its DIR3 code and the Provincia. The form does not say where to find the DIR3 code: ask at your office.
- Submit the form (on paper, original and copy) at the immigration office of the province where the activity starts, in person or through a representative, or electronically (Mercurio) (art. 54.3); the extension, electronically only (art. 55.4). For higher studies, if you are in Spain lawfully, your centre can also file it electronically (art. 54.6). From abroad the visa is requested at the consulate (art. 36.1): ask there which form it requires. Details: student stay.
- If the authorisation or visa lets you stay more than 6 months, the next step is the TIE.
