In short: the EX-17 ("Solicitud de Tarjeta de Identidad de Extranjero") is the official application for the TIE: first card, renewal or duplicate. There is no official EX-17 form in English: the PDF on inclusion.gob.es is free and Spanish only. Fill in 4 sections in capitals, black ink or typed, and submit original and copy.
In person or by representative? In person, mandatory. The form says it is submitted personally to the National Police, not online or at public registries. Your fingerprints are taken there: the TIE carries two fingerprints, mandatory from the age of 6 (Regulation (EC) 1030/2002, art. 4b). The card is handed to the person concerned, who shows they are its intended recipient (art. 209.11 of RD 1155/2024).
1) DATOS DE LA PERSONA EXTRANJERA
- PASAPORTE, N.I.E., 1er Apellido, 2º Apellido, Nombre: passport, NIE, surnames and given name.
- Sexo(1): H man, M woman, X undefined (only if it is recognised for you in your country of origin).
- Fecha de nacimiento(2): dd/mm/aaaa.
- Lugar, País, Nacionalidad: Lugar and País are in the date-of-birth row (place and country of birth; the notes do not explain them).
- Estado civil(3): S single, C married, V widowed, D divorced, Sp separated.
- Nombre del padre, Nombre de la madre, Domicilio en España, Nº, Piso, Localidad, C.P., Provincia, Teléfono móvil, E-mail: parents, address in Spain, contact.
- Representante legal, en su caso, DNI/NIE/PAS, Título(4): only if you have one; title of the representation.
2) DATOS DEL REPRESENTANTE A EFECTOS DE PRESENTACIÓN DE LA SOLICITUD(5): per note (5), only if the filer is not the applicant; if you file, leave it blank. Fields: Nombre/Razón Social, DNI/NIE/PAS, Domicilio en España, Nº, Piso, Localidad, C.P., Provincia, Teléfono móvil, E-mail and Representante legal (DNI/NIE/PAS, Título(4)). The form does not say when someone else may file it and requires filing "de forma personal" with the National Police: ask at your police station before using a representative.
3) DOMICILIO A EFECTOS DE NOTIFICACIONES
- Nombre/Razón Social, DNI/NIE/PAS, domicilio, Teléfono móvil, E-mail: who receives notices and where (if empty, the applicant is notified).
- CONSIENTO (Dehú): you accept notices in the Dirección electrónica habilitada Única; needs a valid electronic certificate or Cl@ve. Outside the obligated groups (legal persons and others), you get notices this way only if you tick it (note 6).
4) DATOS RELATIVOS A LA SOLICITUD(7): in 4.1 TIPO DE DOCUMENTO (art. 209) tick the one that applies: TARJETA INICIAL (first card), RENOVACIÓN DE TARJETA (renewal) or DUPLICADO POR PÉRDIDA, SUSTRACCIÓN, DETERIORO O CAMBIO DE DATOS (duplicate: loss, theft, damage or change of data).
Bottom: place, date and FIRMA DEL SOLICITANTE (yours or, where applicable, your legal representative's). DIRIGIDA A, Código DIR3 and PROVINCIA: addressee and its code; the instructions do not explain them.

Steps:
- Download the official EX-17 (or the editable version) and fill it in as explained above.
- Before signing, avoid the typical mistakes: do not file it online or at a public registry, and do not tick Dehú without an electronic certificate or Cl@ve (you cannot open the notices).
- Print the original and a copy, and sign.
- Gather the rest: grant decision (resolución de concesión), fee modelo 790 code 012 paid, passport (original and copy), a passport-size photo with a white background and, if asked, proof of padrón registration. Full list: how to get the TIE.
- Book a prior appointment for fingerprinting and TIE issuance at the Public Administration e-office (icpplus).
- Attend the appointment in person with everything: your fingerprints are taken.
- Collect the card when it is ready; it is handed to the person concerned (art. 209.11).
